St. Joseph's Home for Children Mental Health Case Management - Catholic Charities of St. Paul and Minneapolis, MN

St. Joe's mental health case management serves children under age 18 with severe emotional disturbances with case management needs. Staff provide the support parents need to make informed decisions about a child’s care.

At St. Joe’s, we use an asset-based approach to build on what works for each child. Mental health case managers are an important part of a child’s therapeutic treatment team. We customize plans to each child and family’s needs.

Mental health case managers support each child’s custom treatment plan by identifying and coordinating the multiple services the child needs. Case managers can arrange physical or mental health care appointments, extra in-home support, transportation to school and help with transition to or from other services.

Hours:
Monday – Friday, flexible hours depending on child's and family's needs
